Driving licence forms

Information about Forms

You can order most of the forms easily and conveniently via: the Swedish Transport Agency

Once you have ordered one or more forms, they will be sent to the address where you are registered. It takes between three and five working days for the forms to reach you. Most forms cost nothing to order, but some charge a small fee. This fee is paid by you via a payment card sent home with your order. About the fee when ordering the form.
Please note - Avin may arrive a few days later than the application form.

Driving licence with health declaration
Form for applying for a driving licence for vehicles such as mopeds, motorcycles and passenger cars. Group I: AM, A1, A, B, BE and Tractor licence. The application for a Group I driving licence must be accompanied by a visual certificate.Read more about Group I Driving Licence and Sight Certificate.

Driving licence with medical certificate
Form for applying for a Group 2 and 3 driving licence.
Group II: AM, A1, A2, A, B, BE, C1, C1E, C and CE licences. Group III: AM, A1, A2, A, B, BE, C1, C1E, C, CE, D1, D1E, D and DE. For the C1, C1E, C, CE, D1, D1E, D and DE licences, you must complete the application with a medical certificate. Read more about Group II and III driving licences and medical certificates.

Application tutorship private practice driving
Application form for approved tutorship for private practice driving. In the case of practice driving of several pupils, an individual application must be made for each pupil. For tutors in private practice driving for qualification B , tutor training must be completed.Read more about Applying for Supervision.

Synintyg
Form to use if you need to submit a supplementary certificate. Read more about Synintyg at Körkortsportalen.

Health declaration with eye examination certificate
Supplementary health declaration and eye test certificate form. More about the health and vision form.

Medical certificate for C, CE, D, DE or Taxi
Medical certificate form for driving licence entitlements C, CE, D, DE or taxi driver's licence. The medical certificate can be used, for example, when applying for renewal of a higher driving licence (lorry and bus).

Medical certificate on alcohol, drugs, medicines
This form is a medical certificate used for the diagnosis of dependence or addiction and for a special medical opinion following a serious drink-driving offence.

Medical certificate Diabetes
Medical certificate form for a diagnosis of diabetes.

Medical certificate of visual function
Form for medical certificate of vision, and must be issued by an eye specialist.

    Intensive course MC

    Our intensive course is for those who can handle a motorcycle.

    The intensive course includes the following training elements:

    Course programme

    (Morning)

    DAY 1
    Monday: 07.00-12.30
    Initial traffic, as well as high speed braking and a safety check

    DAY 2-4
    Tuesday-Thursday: 09.30-12.30
    Driving in traffic

    Day 5
    Friday: 08:20 or 09:30
    Road trip, fixed distance, 11 miles lasting about 3 hours.
    UBK - training check. This training is only booked when the trainer
    the trainer has given the go-ahead.

    Course programme

    (Afternoon)

    DAY 1
    Monday: 10.40-12.40 + 13.40-16.40
    Initial traffic, plus high speed braking and a safety check

    DAY 2-4
    Tuesday-Thursday: 13.40-16.40
    Driving in traffic

    DAY 5
    Friday: 08.20 or 09.30
    Road trip, fixed distance, about 11 miles lasting about 3 hours.
    UBK - training check. This training is only booked when the trainer
    the trainer has given the go-ahead.
